I confirm that I would like to receive CommonSpirit survey invitations via text message (SMS) on my mobile device.   
 
Please enter the 10-digit phone number of the mobile phone you would like the text messages to be sent to. Enter only numbers, with no dashes or spaces (example: 5551234567).   
 
Please enter your CommonSpirit employee ID. If you don’t know it, it’s available on EmployeeCentral.
  • In the top right corner of the EmployeeCentral screen, click the circle that contains your initials or picture.
  • In the drop-down menu, select My Account.
  • On the MyAccount page, select About.
  • On the About page, find your Employee Number (located under your Title). This is your employee ID.

If you also have a Lawson Employee Number (also referred to as a Dignity Health Employee Number), do not use that number to open your survey; be sure to use your Employee Number as described above.
